Uric Acid is Not Just an Indication of Gout

From a functional medicine perspective, uric acid is more than just a marker for gout. It can serve as a clue to metabolic health, oxidative stress, mitochondrial function, kidney clearance, and dietary patterns.

Functional interpretation of uric acid

Low uric acid

Often overlooked, low uric acid may suggest

  • Low protein intake or poor amino acid status

  • Low purine intake

  • Poor liver function or reduced purine production

  • Copper deficiency (less common)

  • Excess oxidative stress, since uric acid is also an antioxidant

  • Rare genetic conditions affecting uric acid metabolism

  • Certain medications, including Allopurinol or uricosuric drugs

Some functional clinicians also consider chronically low uric acid a possible clue to depleted antioxidant reserves in patients with chronic illness.

Optimal functional range

Many integrative clinicians aim for approximately

  • Women: 4 to 5 5 mg/dL

  • Men: 4 5 to 6 mg/dL

Again, these are not universally established medical standards.

High uric acid and metabolism

Even before gout develops, elevated uric acid is associated with

  • Insulin resistance

  • Metabolic syndrome

  • Fatty liver

  • Hypertension

  • Chronic kidney disease

  • Increased cardiovascular risk

Uric acid often rises years before blood sugar becomes abnormal.

Why?

High insulin decreases kidney excretion of uric acid

High fructose intake rapidly increases ATP depletion, producing uric acid as a byproduct

This creates a cycle

Fructose → ATP depletion → increased uric acid → mitochondrial dysfunction → insulin resistance → even higher uric acid

This is why uric acid is increasingly viewed as an early metabolic marker.

Functional root causes

Excess fructose

Especially from

  • Soda

  • Juice

  • High-fructose corn syrup

  • Frequent processed foods

Whole fruit is generally much less problematic because fiber slows absorption.

Insulin resistance

One of the strongest associations.

Often accompanies

  • Elevated triglycerides

  • Low HDL

  • Central obesity

  • Elevated fasting insulin

Poor kidney clearance

Even mild reductions in kidney function increase uric acid.

Alcohol

Beer has the greatest effect because it combines alcohol with purines.

Alcohol also increases lactate, which competes with uric acid for kidney excretion.

High cell turnover

Seen with

  • Psoriasis

  • Chemotherapy

  • Rapid weight loss

  • Some blood disorders

Obstructive sleep apnea

Intermittent oxygen deprivation increases ATP breakdown and uric acid production.

Nutrition considerations

Increase

  • Hydration

  • Vegetables

  • Vitamin C-rich foods

  • Dairy if tolerated

  • Weight training

  • Insulin sensitivity

Limit if elevated

  • Sugar-sweetened beverages

  • Excess alcohol

  • Ultra-processed foods

  • Large fructose loads

Purine-rich foods like red meat have a much smaller impact on uric acid than sugary beverages in most people.

Functional workup if uric acid is abnormal

Rather than looking at uric acid alone, consider evaluating

  • Fasting insulin

  • Hemoglobin A1c

  • Fasting glucose

  • Triglyceride-to-HDL ratio

  • Liver enzymes

  • Kidney function

  • High-sensitivity CRP

  • Ferritin

  • Vitamin C intake

  • Body composition

  • Blood pressure

Supporting metabolism naturally

Depending on the underlying cause, interventions may include

  • Improving insulin sensitivity through resistance training and protein-forward nutrition

  • Reducing added sugars, particularly fructose-containing beverages

  • Achieving healthy weight loss gradually rather than through crash dieting

  • Ensuring adequate hydration

  • Optimizing sleep and screening for sleep apnea if indicated

  • Increasing vitamin C intake through diet or supplementation when appropriate
